NEWBORN REGISTRATION INFORMATION FOR NEWBORNS DELIVERED OFF-BASE
1.    Parents must call Patient Administration at 850-883-8251, Monday-Friday, 7 a.m. to 4 p.m., or 850-418-7268 nights and weekends, to register newborns in the electronic health record. 
2.    Patient Administration will provide the parents with a DOD ID number for the newborn. 
3.    Parents must pick up their newborn’s birth certificate at a local tax collector’s office. It is usually available five business days after birth. 
4.    Parents must bring the DOD ID number and birth certificate to DEERS/ID facility (MPF) at the military base. 
a.    Warning: Not informing DEERS of the DOD ID number could result in multiple DOD IDs for the newborn. 
5.    Parents must then call TRICARE at 800-444-5445 to enroll their infant in TRICARE Prime or TRICARE Select. 
6.    A Social Security card should be mailed to the home address provided to the hospital at time of birth within 6-8 weeks. 

TWO DAY FOLLOW UP FOR NEWBORNS DELIVERED OFF-BASE
1. Complete Steps 1 and 2 above to register the newborn in the electronic health record. 
2. You MUST schedule a newborn follow up appointment NO MORE than 2 days after discharge, including weekends. PLEASE CALL BEFORE/ON DAY OF DISCHARGE FROM OUTSIDE FACILITY: WEEKDAYS: 850-883-9501; WEEKENDS, NIGHTS, AND HOLIDAYS: 850-883-8361

The in-hospital (main) pharmacy fills new prescriptions after an appointment with an Eglin hospital provider. All beneficiaries, regardless of status, must use the satellite pharmacy for all prescription refills and for new scripts placed from off-base providers.
 

• QANYWHERE (TEXT): Patients can text the pharmacy to activate prescriptions.
  1. Patients may text “Get in Line” to 1-833-274-5398 and follow the prompts
  2. Patients may utilize QAnywhere for both on-base script activations (main pharmacy) and off-base activations sent via e-scripts (Satellite Pharmacy)

If you are out of refills on your prescription, you will require a renewal.Renewals must be placed by your Primary Care Manager or the originating provider who ordered the script.Depending on the medication and stability on the medication, you may have to be seen for follow-up.If the script was written by a 96th MDG provider, you may request a renewal by messaging the provider and their team on the MHS GENESIS Patient Portal. If you are having issues logging on to the patient portal, you can place a message to your provider by calling the Central Appointments Line at 850-883-8600 and selecting the option for your clinic.
 

The 96 MDG is not currently scheduled to close on any AFMC Family Days.
 

Starting June 5, the 96th Medical Group outpatient clinics will be closed every Wednesday, 7 a.m. to 9:15 a.m., for medical readiness training. The Emergency Department, Pharmacy, Lab, and Inpatient departments will remain open during this time
