Immunizations 

Routine immunizations are available by appointment and walk-in. Active duty service members and scheduled appointments receive priority. To schedule:

Florida Form DH680 (Florida Blue Card)

Please initiate requests at the immunization front desk self-check-in kiosk and bring your child's current immunization record. If vaccines are required, they must be administered prior to issuing the form. Standard processing takes three business days. A same-day walk-in option is available, though wait times may be significantly longer and require all records to be current. Standard MHS GENESIS immunization records can be printed the same day via the self-check-in kiosk, Medical Records, or any primary care clinic within the Medical Group.

Walk-Ins

Walk-in patients may present without an appointment for the following:
  • Active duty service members with PCS/deployment orders
  • Active duty service members with red or yellow IMR vaccine requirements
  • Beneficiaries requiring rabies or tetanus vaccines after exposure/injury
  • Pregnant women for Tdap or RSV vaccines
  • Medical readiness patients (includes CDC onboarding, fire department, hospital staff, and police)

Note: Priority walk-in status is granted to Active-Duty Service Members with overdue (red) IMR requirements or PCS/out-processing checklists. Please note that HIV IMR requirements require a lab draw, not an immunization visit.

Active duty service members can print vaccine records by visiting MyIMR:

Adult patients with an MHS GENESIS Patient Portal account may view and print their vaccine records by logging into the portal and selecting Health & Immunizations.

COVID-19 Vaccine 

FDA-approved or authorized COVID-19 vaccines are currently available to all U.S. citizens and residents. Visit the CDC website for more information.

Getting the COVID-19 Vaccine 

  • The Eglin AFB Immunizations Clinic offers the Pfizer vaccine for patients aged 12 and older.
  • The Moderna vaccine is available by request for patients aged 6 months to 12 years.
  • Please call 850-883-9082 to place a request.
