Family Health

We provide primary medical care needs to retirees, family members of retirees, and family members of active duty service members. If you have not seen your primary care manager in over a year, please schedule an appointment to see your provider.

When you see your primary care manager, please:

  1. Bring a current medication list with you
  2. Let your provider know what medication refills are needed
  3. Bring a log of your blood sugars if you are diabetic
  4. Bring a log of your blood pressures if you have hypertension (high blood pressure)
  5. Make a prioritized list of any concerns or questions

  • Pregnancy Testing

Important Information

We are not currently offering any walk-in services due to manning shortages.

Further, we do not offer walk-in infant weight checks, or adult/pediatric vitals checks.
The Family Health Clinic is a COVID Self-Test Kit distribution site for empaneled beneficiaries. Each beneficiary can receive up to 8 kits per calendar month. Please call the appointment line for details
